Your First Visit
Prior to your first office visit, we will send you a copy of our health history form for you to have ready at your initial consultation. Alternatively, you can print the form online from our web site.Our New Patient Coordinator will greet you when you arrive and will escort you to a private consultation room where she will review your health and dental history and you can discuss your main concerns. X-rays of your teeth and profile are taken prior to the doctor's exam so we can be aware of the state of your dentition and the relationship of your bite. Then Dr. Petty, Dr. Bielik or Dr. Burke will perform a complete orthodontic examination and discuss your treatment options and treatment timing. We will review the function of various orthodontic appliances that may be considered for your treatment and allow you to chose titanium (metal) or ceramic (clear) braces. If treatment is indicated, our New Patient Coordinator will complete your diagnostic records by taking dental impressions, including a wax bite, and photographs of your teeth and face. Upon review of your records, the doctors will finalize your treatment plan and send both you and your dentist a copy. Appointments & Scheduling We have put much effort into designing our scheduling system. Our goal is to allow you to be seen on time for your appointment, do all necessary scheduled treatments, make appointments as stress-free for you as possible and minimize interruptions to your busy schedule. We strive to accommodate school and work schedules the best that we can. Our advanced computer system helps us to integrate scheduling and diagnostic data so that we can minimize your waiting time and complete your treatment on schedule. Most people seeking orthodontic treatment have important obligations during the day, usually involving school or work. When active orthodontic treatment begins, each patient is seen approximately every six to eight weeks and some of these appointments my conflict with work or school schedules. Please allow the following allotted time for each procedure.
Financial Information Costs for orthodontic treatment vary depending on the type of treatment required. Our Financial Coordinator will explain our payment options and help you decide which one is best for you. We offer discounts for multiple family members and interest-free payments as a courtesy to our patients. We accept most major credit cards and can automatically charge your account monthly. Also, payments can be made through your own personal on-line account. When you provide us with your insurance information at the new patient appointment or prior to your initial visit, we will be happy to call your insurance company to verify your benefits. Then we can arrange a payment plan where the estimated insurance benefits are coordinated with the patient's financial responsibility. For your convenience, our Insurance Coordinator will submit both predetermination and insurance claims pertaining to charges for care rendered by our office. However, please be aware that our primary relationship is with patients or their families and not with their respective insurance companies. Please bring your claim form to the initial visits and complete the employee portion of the form and sign where indicated. CareCredit is only accepted in our Oak Lawn office. ![]() |
